WHEREAS, adverse action was taken against the Massage Practitioner license application submitted by Deana M. Rousseau (License ID #20110003355) for the premises located at 2065 Randolph Avenue in Saint Paul by Notice of Intent to Deny License dated July 13, 2012, alleging the licensee failed to submit the following: 1) a current certificate of General Liability insurance showing the City of Saint Paul as additional insured and 2) a notarized copy of a current national certification in Therapeutic Massage and Bodywork or successful completion of a written and practical examination from the City of Saint Paul authorized examiner; and
WHEREAS, the licensing office recommended license denial; and
WHEREAS, the licensee did not respond to the Notice of Intent to Deny License to submit the required information, request a hearing or withdraw her license application; and
WHEREAS, the Notice of Intent to Deny License stated that if the licensee failed to submit the required information, request a hearing or withdraw her license application by July 23, 2012, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, that the Massage Practitioner license application submitted by Deana M. Rousseau is hereby denied.
