Approving adverse action against the Tree Trimmer & 1 Vehicle license held by Michael P. Haege, d/b/a Custom Cut.

 

Body

WHEREAS, the Tree Trimmer & 1 Vehicle license held by Michael P. Haege d/b/a Custom Cut (License ID #20120002225) for the City of Saint Paul, was the subject of a Notice of Intent to Suspend License dated November 8, 2013, alleging licensee failed to pay delinquent license and late fees of $267.00, schedule an inspection of his vehicle(s) and submit the following information: 1) current vehicle information; 2) obtain a sticker from DSI showing the expiration month and date; 3) current certificate of General Liability insurance; 4) proof of registration with the Commissioner of Agriculture; 5) Proof of compliance with the American National Standards Institute (ANSI); 6) proof that they have employed an individual who possesses current certification as an arborist with the International Society of Arboriculture; and 

 

WHEREAS, per Saint Paul Legislative Code §310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information in order to maintain the license; and  

 

WHEREAS, the licensee failed to respond to the Notice of Intent to Suspend License to pay the delinquent license and late fees, schedule a vehicle inspection and submit the required information or request a hearing; and

 

WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to pay the delinquent license and late fees, schedule a vehicle inspection, submit the required information, request a hearing or cancel his license by November 18, 2013, the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it

 

RESOLVED, the Tree Trimmer & 1 Vehicle license held by Michael P. Haege d/b/a Custom Cut is hereby suspended for failure to pay delinquent license and late fees of $267.00 and have his vehicle inspected.

 

FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information in order to maintain the license.

 

The license will remain suspended until all license fees and penalties are paid in full, his vehicle is inspected and all requested documents are submitted to the Department of Safety and Inspections.
