Body
WHEREAS, the Massage Center-A (Commercial) license held by Renu Massage & Bodywork, LLC d/b/a Renu Massage & Bodywork (License ID #20120001480) for the premises located at 246 Albert Street South, Ste. 2B in Saint Paul was the subject of a Notice of Intent to Suspend License dated June 13, 2018; and

WHEREAS, the licensee failed to pay delinquent license and late fees and submit the following required information: list of massage practitioners operating on site with license numbers and 3) a completed Certificate of Compliance Minnesota Workers’ Compensation Law form; and

WHEREAS, per Saint Paul Legislative Code §310.05 (m) (2), the licensing office recommended suspension of the license and imposition of a $500.00 matrix penalty for failure to submit required information to maintain the license; and

WHEREAS, the licensee did not respond to the Notice of Intent to Suspend Licenses to pay the delinquent license and late fees and submit the required information, request a public hearing or cancel the license; and

WHEREAS, the Notice of Intent to Suspend Licenses stated that if the licensee failed to pay the delinquent license and late fees and submit the required information, request a public hearing or cancel the license by June 25, 2018, that the matter would be placed on the next available Council consent agenda to impose the recommended penalty; and

WHEREAS, a Notice of Council Hearing was sent to the licensee on July 6, 2018, notifying her that DSI’s recommendation to suspend her Massage Center-A (Commercial) license and impose a $500.00 matrix penalty will be calendared for the July 18, 2018 Consent Agenda; now, therefore, be it
