Approving adverse action against the Auto Repair Garage and Auto Body Repair/Painting Shop licenses held by Cha Sur Vang, d/b/a Solange Auto Service at 291 Fillmore Avenue East.
Body
WHEREAS, the Auto Repair Garage and Auto Body Repair/Painting Shop licenses held by Cha Sur Vang d/b/a Solange Auto Service (License ID# 20010005008) for the premises located at 291 Fillmore Avenue East in Saint Paul were the subject of a Notice of Intent to Suspend Licenses dated August 22, 2018, stating the licensee failed to pay delinquent license and late fees and submit a completed Certificate of Compliance Minnesota Workers’ Compensation law form; and
WHEREAS, per Saint Paul Legislative Code §310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit the required information to renew the license; and
WHEREAS, the licensee did not respond to the Notice of Intent to Suspend Licenses to pay the delinquent license and late fees and submit the required information, request a public hearing or cancel the licenses; and
WHEREAS, the Notice of Intent to Suspend Licenses stated that if the licensee failed to pay delinquent license and late fees and submit required information, request a public hearing or cancel the licenses by August 31, 2018 that the matter would be placed on the September 12, 2018 consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Auto Repair Garage and Auto Body Repair/Painting Shop licenses held by Cha Sur Vang d/b/a Solange Auto Service (License ID# 20010005008) for the premises located at 291 Fillmore Avenue East in Saint Paul are hereby suspended.
FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information to renew the license.
FINALLY RESOLVED, the licenses will remain suspended until the all delinquent license and late fees are paid and required information is submitted to the Department of Safety...
