WHEREAS, the City Contractor-General Installation license held by Selby Ornamental Iron Co. d/b/a Selby Ornamental Iron Co. (License ID# 00TR200310) for the premises located at 1160-7th Street East in Saint Paul was the subject of a Notice of Intent to Suspend License dated August 22, 2018, stating the licensee failed to submit current General Liability insurance with a 30-day notice of cancellation and naming the City of Saint Paul as additional insured; and
WHEREAS, per Saint Paul Legislative Code §310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit the required insurance information to renew the license; and
WHEREAS, the licensee did not respond to the Notice of Intent to Suspend License to submit the required insurance information, request a public hearing or cancel the license; and
WHEREAS, the Notice of Intent to Suspend License stated that if the licensee failed to submit the required insurance information, request a public hearing or cancel the licenses by August 31, 2018 that the matter would be placed on the September 12, 2018 consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the City Contractor-General Installation license held by Selby Ornamental Iron Co. d/b/a Selby Ornamental Iron Co. (License ID# 00TR200310) for the premises located at 1160-7th Street East in Saint Paul is hereby suspended.
FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required insurance information to renew the license.
FINALLY RESOLVED, the license will remain suspended until the required insurance information is submitted to the Department of Safety and Inspections.
