WHEREAS, the Massage Practitioner license application submitted by Tamara Jean Hillis (License ID# 20180000375) for the premises located at 762 Cleveland Avenue South in Saint Paul was the subject of a Notice of Intent to Deny License dated August 20, 2018, and
WHEREAS, the Notice stated the licensee failed to submit a completed and notarized Personal Affidavit required to continue the application process; and
WHEREAS, the licensee did not respond to the Notice of Intent to Deny License to submit a completed and notarized Personal Affidavit, request a public hearing or withdraw the license application; and
WHEREAS, the Notice of Intent to Deny License stated that if the licensee failed to submit a completed and notarized Personal Affidavit, request a public hearing or withdraw the license application by August 30, 2018 that the matter would be placed on the September 12, 2018 consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Massage Practitioner license application submitted by Tamara Jean Hillis (License ID# 20180000375) for the premises located at 762 Cleveland Avenue South in Saint Paul is hereby denied.
