Approving adverse action against the Auto Repair Garage license application submitted by HAP Transportation, d/b/a HAP Transportation at 44 Acker Street East.
Body
WHEREAS, the Auto Repair Garage license application submitted by HAP Transportation d/b/a HAP Transportation (License ID# 20150002227) for the premises located at 44 Acker Street East in Saint Paul was the subject of a Notice of Intent to Deny License dated March 30, 2018; and
WHEREAS, on May 24, 2017, the applicant was sent a letter from the Department of Safety and Inspections (DSI) stating the application was incomplete and a site plan review application needed to be submitted to DSI for review; and
WHEREAS, the applicant contacted DSI on April 10, 2018 to request an extension in order to complete the site plan review application; and
WHEREAS, the applicant was given a two week extension to April 24, 2018; and
WHEREAS, the applicant failed to follow-up with DSI and submit the site plan review application by the April 24, 2018 deadline; and
WHEREAS, the Notice of Intent to Deny License stated that if the applicant failed to submit a completed site plan review application, request a public hearing or withdraw his license application by April 9, 2018, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Auto Repair Garage license application submitted by HAP Transportation d/b/a HAP Transportation (License ID# 20150002227) for the premises located at 44 Acker Street East in Saint Paul is hereby denied.
