Title

Approving adverse action against the MAD-Game Machine and MAD Music Machine licenses held by American Amusement Arcades (License ID #0076265).

 

Body

WHEREAS, the MAD-Game Machine and MAD Music Machine licenses held by American Amusement Arcades d/b/a American Amusement Arcades (License ID #0076265) for the City of Saint Paul were the subject of a Notice of Intent to Suspend Licenses dated December 21, 2017, stating the licensee failed to pay delinquent license and late fees and submit information regarding the type and location of each machine; and

 

WHEREAS, DSI records show on December 18, 2017, the licensee paid the delinquent license fees of but failed to pay late fees of $251.00 and submit the required machine information; and

 

WHEREAS, per Saint Paul Legislative Code §310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information to maintain the license; and 

 

WHEREAS, the licensee did not respond to the Notice of Intent to Suspend Licenses to submit the required machine information, request a public hearing or cancel the licenses; and

 

WHEREAS, the Notice of Intent to Suspend Licenses stated that if the licensee failed to submit the required information, request a public hearing or cancel the licenses by January 2, 2018, that the matter would be placed on the next available consent agenda to impose the recommended penalty; now, therefore, be it

 

RESOLVED, the MAD-Game Machine and MAD Music Machine licenses held by American Amusement Arcades d/b/a American Amusement Arcades (License ID# 0076265) for the City of Saint Paul are hereby suspended.  

 

FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required machine information in order to maintain the licenses.

 

FINALLY RESOLVED, the licenses will remain suspended until the required machine information is submitted to the Department of Safety and Inspections.
