Approving adverse action against the Tree Trimmer & 1 Vehicle, and Tree Trimmer-Each Additional Vehicle licenses held by Twin City Tree Service, Inc.

WHEREAS, the Tree Trimmer & 1 Vehicle and Tree Trimmer-Each Additional Vehicle licenses held by Twin City Tree Service, Inc. d/b/a Twin City Tree Service, Inc. (License ID #20150003603) for the City of Saint Paul were the subject of a Notice of Intent to Suspend Licenses dated May 2, 2017, stating the licensee failed to pay delinquent license and late fees and submit the following information to maintain the licenses 1) current vehicle information; 2) either a current DOT inspection forms or schedule an appointment with DSI for vehicle inspection(s); 3) a current certificate of General Liability insurance showing coverage from April 12, 2017 to April 12, 2018; and
WHEREAS, per Saint Paul Legislative Code §310.05 (m) (2), the licensing office also recommended a $500.00 matrix penalty for failure to submit required information to maintain the license; and
WHEREAS, the licensee did not respond to the Notice of Intent to Suspend Licenses to pay the delinquent license and late fees and submit the required information; and
WHEREAS, the Notice of Intent to Suspend Licenses stated that if the licensee failed to pay the delinquent license and late fees and submit the required information or request a public hearing by May 12, 2017, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Tree Trimmer & 1 Vehicle and Tree Trimmer-Each Additional Vehicle licenses held by Twin City Tree Service, Inc. d/b/a Twin City Tree Service, Inc. (License ID #20150003603) are hereby suspended for failure to pay delinquent license and late fees.
FURTHER RESOLVED, the licensee is ordered to pay a matrix penalty of $500.00 for failure to submit required information in order to maintain the license.
FINALLY RESOLVED, the licenses will remain suspended until all delinquent license fees and penalties are paid in full and all required information is submitted to the Department of Safety and Inspections.
