Approving adverse action against the Auto Repair Garage license application submitted by Sandra Medina, d/b/a Rudy’s Auto Repair at 620 Rice Street.

WHEREAS, the Auto Repair Garage license application submitted by Sandra Medina d/b/a Rudy’s Auto Repair (License ID #20150001566) for the premises located at 620 Rice Street in Saint Paul was the subject of a Notice of Intent to Deny License dated June 7, 2016, stating the licensee’s application was still pending and DSI could not continue processing her application until she submitted the required information; and
WHEREAS, DSI sent the license applicant an email on October 15, 2015 and a follow-up letter on April 22, 2016, listing the required information needed to allow them to continue the application process; and
WHEREAS, the licensee did not respond to the Notice of Intent to Deny License to submit the required information to continue the application process, request a public hearing or withdraw her license application; and
WHEREAS, the Notice of Intent to Deny License stated that if the licensee failed to submit the required information to continue the application process, request a public hearing or withdraw her license application by June 17, 2016 that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Auto Repair Garage license application submitted by Sandra Medina d/b/a Rudy’s Auto Repair is hereby denied.
