WHEREAS, the Massage Practitioner license application submitted by Sara A. Lundequam (License ID# 20140002647) for the premises located at 493 Selby Avenue in Saint Paul was the subject of a Notice of Intent to Deny License dated January 26, 2016, stating her application was in “pending” status until she submitted the following information: 1) a current certificate of General and Professional Liability insurance showing minimum coverage of $1,000,000 each with a 30-day notice of cancellation and naming the City of Saint Paul as additional insured and 2) proof of affiliation with a City of Saint Paul licensed massage center or state licensed health facility; and
WHEREAS, the licensing office then recommended denial of her Massage Practitioner license; and
WHEREAS, the applicant failed to respond to the Notice of Intent to Deny License to submit the required information in order to continue the application process; and
WHEREAS, the Notice of Intent to Deny License stated that if the applicant failed to submit the required information or request a public or administrative hearing by February 5, 2016, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Massage Practitioner license application submitted by Sara A. Lundequam is hereby denied.
