WHEREAS, the Taxicab Driver (Provisional) license application submitted by Dominique Antoinette Tucker (License ID #20140003634) for the City of Saint Paul was the subject of a Notice of Intent to Deny License dated April 16, 2015, stating that the applicant failed to provide a copy of her State of Illinois driving record required by Saint Paul Legislative Code ยง 376.16 (e) (5) (e); and
WHEREAS, as a result, the licensing office recommended license denial; and
WHEREAS, the applicant contacted DSI after receiving the Notice of Intent to Deny License to request an extension in order to obtain her State of Illinois driving record which was granted; and
WHEREAS, the applicant has failed to follow-up with DSI since the extension was granted to submit a copy of her State of Illinois driving record;
WHEREAS, the Notice of Intent to Deny License stated that if the applicant failed to submit her State of Illinois driving record, request a public hearing or withdraw her license application by April 27, 2015, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, that the Taxicab Driver (Provisional) license application submitted by Dominque Antoinette Tucker is hereby denied.
