Approving adverse action against the Sidewalk Contractor license application submitted by Valley Concrete, Inc.
Body
WHEREAS, the Sidewalk Contractor license application submitted by Valley Concrete, Inc. d/b/a Valley Concrete, Inc. (License ID #20140001006) for the City of Saint Paul was the subject of a Notice of Intent to Deny License dated May 15, 2015, stating that the applicant had not obtained approval from the Department of Public Works and provided an original Surety Bond in the amount of $5,000.00 which was needed to finish processing the application; and
WHEREAS, the licensing office recommended license denial; and
WHEREAS, the applicant did not respond to the Notice of Intent to Deny License to submit the required information, request a public or administrative hearing or withdraw the license application; and
WHEREAS, the Notice of Intent to Deny License stated that if the applicant failed to submit the required information, request a public or administrative hearing or withdraw the license application by May 25, 2015, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Sidewalk Contractor license application submitted by Valley Concrete, Inc. d/b/a Valley Concrete, Inc. is hereby denied.
