WHEREAS, the Massage Center-A (Commercial) license application submitted by Angela Rucinski, LLC d/b/a Angela Rucinski (License ID #20140003808) for the premises located at 2500 University Avenue West, Unit C8 in Saint Paul was the subject of a Notice of Intent to Deny License dated May 15, 2015, stating that the applicant had not obtained a DSI Environmental Inspection approval and provided a copy of a signed lease or purchase agreement for the property needed to finish processing the application; and
WHEREAS, the licensing office recommended license denial; and
WHEREAS, the applicant did not respond to the Notice of Intent to Deny License to submit the required information, request a public or administrative hearing or withdraw her license application; and
WHEREAS, the Notice of Intent to Deny License stated that if the applicant failed to submit the required information, request a public or administrative hearing or withdraw her license application by May 25, 2015, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Massage Center-A (Commercial) license application submitted by Angela Rucinski, LLC d/b/a Angela Rucinski is hereby denied.
