Approving adverse action against the Food/Institutional Facility license application submitted by Al-Ihsan Islamic Center for 955 Minnehaha Avenue West.

WHEREAS, adverse action was taken against the Food/Institutional Facility license application submitted by Al-Ihsan Islamic Center d/b/a Al-Ihsan Islamic Center (License ID #20100004200) for the premises located at 955 Minnehaha Avenue West in Saint Paul, by Notice of Intent to Deny Licenses dated January 25, 2013, alleging the licensee failed to notify the Department of Safety and Inspections (DSI) of the fact that they put a kitchen project on hold due to lack of funding and did not complete and return a Request to Withdraw/Request for Refund form to DSI; and
WHEREAS, the licensing office recommended license denial; and
WHEREAS, the licensee did not respond to the Notice of Intent to Deny Licenses to submit a completed Request to Withdraw/Request for Refund form, request a hearing or withdraw the license application; and
WHEREAS, the Notice of Intent to Deny Licenses stated that if the licensee failed to submit a completed Request to Withdraw/Request for Refund form, request a hearing or withdraw the license application by February 4, 2013, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, that the Food/Institutional Facility license application submitted by Al-Ihsan Islamic Center d/b/a Al-Ihsan Islamic Center is hereby denied.
