WHEREAS, adverse action was taken against the Massage Practitioner license application submitted by Jonathan Eric Wagner d/b/a Jonathan Eric Wagner (License ID #20120001361) for the premises located at 1053 Grand Avenue, Ste. #101 in Saint Paul, by Notice of Intent to Deny License dated August 22, 2012, alleging licensee failed to submit proof of affiliation with Avivage Massage; and
WHEREAS, the licensee did not respond to the Notice of Intent to Deny License to submit the required information, request a hearing or withdraw his license application; and
WHEREAS, the Notice of Intent to Deny License stated that if the licensee failed to submit the required information, request a hearing or withdraw his license application by September 3, 2012, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, that the Massage Practitioner license application submitted by Jonathan Eric Wagner d/b/a Jonathan Eric Wagner is hereby denied for failure to submit proof of affiliation with Avivage Massage.
