Approving adverse action against the Food Vending Machine, Food Vending Machine Operator, and Food Vehicle license applications submitted by Momtaz Mahayni, d/b/a A And M M Corp.

WHEREAS, adverse action was taken against the Food Vending Machine, Food Vending Machine Operator and Food Vehicle license applications submitted by Momtaz Mahayni d/b/a A And M M Corp (License ID #20110002881) for the City of Saint Paul by Notice of Intent to Deny Licenses dated July 11, 2012, alleging the licensee failed to contact DSI to schedule an Environmental Facility Inspection, an Environmental Vehicle Inspection and submit the vehicle license number required if transporting food, goods, etc; and
WHEREAS, the licensing office recommended license denial; and
WHEREAS, the licensee did not respond to the Notice of Intent to Deny Licenses to schedule the required inspections and submit the vehicle license number, request a hearing or withdraw his license application; and
WHEREAS, the Notice of Intent to Deny Licenses stated that if the licensee failed to schedule the required inspections and submit the vehicle license number, request a hearing or withdraw his license application by July 23, 2012, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, that the Food Vending Machine, Food Vending Machine Operator and Food Vehicle license applications submitted by Momtaz Mahayni d/b/a A And M M Corp are hereby denied.
