WHEREAS, the Massage Practitioner license held by Carl D. De Los Reyes (License ID #20100003636) for the premises located at 2145 Ford Parkway in Saint Paul, was the subject of a Notice of Intent to Revoke License dated March 7, 2014, stating the licensee failed to pay delinquent license and late fees of $117.00 and submit the following required documents: 1) current certificates of General and Professional Liability insurance with a 30-day notice of cancellation and naming the City of Saint Paul as additional insured; 2) Proof of Affiliation from a City of Saint Paul licensed therapeutic massage center and 3) a copy of his Tax ID or Social Security Number.
WHEREAS, the licensee did respond to the Notice of Intent to Revoke License starting he was no longer interested in operating as a Massage Practitioner in Saint Paul; and
WHEREAS, the licensee was instructed to send a letter to the Department of Safety and Inspections stating he was no longer interested in operating in Saint Paul; and
WHEREAS, the licensee failed to follow-up after the telephone conversation and send the letter to the Department of Safety and Inspections; and
WHEREAS, the Notice of Intent to Revoke Licenses stated that if the licensee failed to pay the delinquent license and late fees and submit required documents, request a public or administrative hearing or notify DSI that he no longer wanted to operate in Saint Paul by March 17, 2014, that the matter would be placed on the consent agenda to impose the recommended penalty; now, therefore, be it
RESOLVED, the Massage Practitioner license held by Carl D. De Los Reyes for the premises located at 2145 Ford Parkway in Saint Paul is hereby revoked.
